Disability-Competent Care Self-Assessment Tool

The purpose of the Disability-Competent Care Self-Assessment Tool (DCCAT) is to help health plans and health systems evaluate their present ability to meet the needs of adults with functional limitations and to identify strategic opportunities for improvement. Resources for Integrated Care has worked with a variety of experts to identify promising practices to better serve these individuals with complex care needs.

The DCCAT has been reworked into a new format that highlights the seven pillars of the Disability-Competent Care model. This new version of the tool allows users to more narrowly assess their disability competence and identify actionable areas for improvement. The DCCAT is available to download as a PDF file. We have also made available two accompanying documents to help you and your organization effectively work through the self-assessment:

  • The DCCAT User Guide (PDF) provides a step-by-step tutorial on how to use the DCCAT and interpret its results
  • The DCCAT Evaluation Results Form (Excel file) provides a means for gathering input from your assessment team and tallying your responses across pillars and sub-sections.
