ACL and the Centers for Medicare and Medicaid (CMS) are hosting a monthly webinar series to assist states and stakeholders in developing high quality programs to address social determinants of health (SDOH) and provider capacity building in home- and community-based services (HCBS). The series will focus on best practices in a number of key areas, including person-centered planning, guardianship reform, aligning HCBS policy with SDOH, housing, employment, community inclusion, and transportation, and more. 

The first webinar, Just Wanna Live My Best Life – Shaping Medicaid HCBS to Fit the Individual Priorities of Self-Advocates” on Thursday, January 23, at 3 – 4:30 PM ET will focus on best practices and participant perspectives on person-centered planning, informed choice in HCBS options, and supported decision making. 

The following questions will be addressed:

  • What is it that consumers are looking for in the PCP and case management process? 
  • In the services they receive and the people providing the supports? 
  • What are some experiences that self-advocates have had with various providers or the Medicaid system that were really strong, positive experiences, and what experiences were negative and produced poor outcomes?
  • What can states and providers learn from consumers, and how can/should they more meaningfully engage and empower individuals within the planning, service delivery, and quality improvement process? 

The webinar will include a panel of people with lived experience, as well as state HCBS program representatives who will share their perspectives on effective practices, policies, and program operations in addressing these topics. There will be ample time for questions and discussion.
