Resources for Integrated Care (RIC) is launching a third Integrated Care Community of Practice (ICCoP) focused on care coordination. This shared learning opportunity for health plan staff explores strategies health plans may use to engage members in care coordination. This ICCoP is open to health plan care coordinators, care managers, administrators, staff engaged in managing member outreach, and other staff responsible for care manager recruitment and retention. The ICCoP is organized and facilitated by faculty from RIC and Community Catalyst.
