The Partnership to Align Social Care is a cross-sector effort to co-design a multi-faceted strategy to enable successful partnerships between healthcare organizations and networks of community-based organizations (CBOs) and Community Care Hubs.

Join the Partnership and health and social care sector leaders for the first in a series of webinars offering a deep dive into the roles and responsibilities of advanced Community Care Hubs improving alignment between health and social care services.

This first of three webinars will provide a deep dive into the core components of CCH leadership and governance structure as well as strategic business development activities.

  • A brief overview of the Community Care Hub model and background on the evolution of the Community Care Hub concept and its value proposition;
