Depression and anxiety are common mental health conditions that can be concurrent with diabetes. This webinar will focus on the value of integrated care to provide rapid identification and treatment of these mental health conditions in the primary care setting.

Objectives:
1. Identify at least 2 screening tools for anxiety and depression.
2. Examine the role of behavioral health providers in working with patients with diabetes.
3. Apply the use of measurement-based care in monitoring depression and anxiety care outcomes.

Credits: 1 hour of ACCME, ANCC, AAPA, ASWB, APA, ACPE, CPEU
To receive a certificate of continuing education, you must electronically sign-in for the session, participate in the entire live internet activity and complete the online evaluation.
