Pillar VI: Long-Term Services and Supports

Long-Term Services and Supports (LTSS) enable participants with disabilities to integrate and function within the community as they choose. Integrating LTSS into the participant’s comprehensive plan of care changes the focus from a traditional illness-based medical model to a person-centered and directed model. The participant’s comprehensive care plan supports the care and life choices. LTSS includes home health supports, transportation, personal care attendants, behavioral health, long-term care, and nursing facilities.
