Primary care is team based, focused on prevention and aims to deliver a timely response to newly emerging episodes of illness. As part of the interdisciplinary care team (IDT) primary care involves a participant-centered approach that focuses on the eventual goal of supporting individuals to achieve maximum function. Particularly for participants with disabilities, primary care involves managing common secondary complications, preventing avoidable hospitalizations, and establishing protocols to facilitate successful transitions between providers and sites of care.
