Individuals living with disabilities who are dually eligible for Medicare and Medicaid are at higher risk for poor health outcomes when compared to individuals without disabilities.[1] This conversation, in collaboration with ICS, provided a platform for questions to be answered on how health plans and providers can improve health outcomes for this population by addressing gaps in care coordination that often disrupt continuity of care and lead to fragmentation of care.
This conversation focused on the key elements of and strategies to improve care coordination, including the role of interdisciplinary teams.
