Case Study: Health Plan and Primary Care Coordination

This case study describes how a health plan completed the Disability Competent Care Self-Assessment Tool (DCCAT) and then developed and carried out an improvement plan aimed at care coordination for their members with disabilities.  The health plan’s work described in this case study, while still under development, contains lessons learned that may benefit other health plans.
